Bible Verses

Romans 3:23: For all have sinned and fall short of the glory of God.
1 John 1:8: If we claim to be without sin, we deceive ourselves and the truth is not in us.
James 4:17: If anyone, then, knows the good they ought to do and doesn't do it, it is sin for them.
Romans 5:12: So sin came into the world through one man, and death through sin, and in this way death came to all men, because all sinned. 
1 Corinthians 6:9-10: Do you not know that the unrighteous will not inherit the kingdom of God? Do not be deceived: No immoral person, idolater, adulterer, male prostitute, homosexual, thief, drunkard, or extortioner will inherit the kingdom of God.
Galatians 5:19-21: Now the deeds of the flesh are evident: sexual immorality, impurity, lustfulness, idolatry, sorcery, hostility, quarreling, jealousy, fits of rage, selfish ambition, arguments, divisions, envy, drunkenness, orgies, and the like. I warn you, as I have been warning you, that those who practice such things will not inherit the kingdom of God.
Hebrews 10:26: For if we deliberately keep on sinning after we have received the knowledge of the truth, there is no longer any sacrifice for sins, but only a fearful expectation of judgment, and a fiery wrath that will consume the adversaries.
1 Peter 2:24: He himself bore our sins in his body on the tree, that we might die to sin and live to righteousness. By his wounds you have been healed.
Proverbs 28:13: Whoever conceals their sins does not prosper, but whoever confesses and renounces them finds mercy.

Prayer

Heavenly Father, I come before you, weighed down by my past sins. I acknowledge my mistakes, and I ask for your forgiveness. Help me to also forgive those who have wronged me, as carrying the weight of resentment harms me more than it affects them. I am sorry for my sins with all my heart. In choosing to do wrong and failing to do good, I have sinned against you whom I should love above all things. I firmly intend, with your help, to do penance, to sin no more, and to avoid whatever leads me to sin. Our Savior Jesus Christ suffered and died for us. In his name, my God, have mercy. I am heartily sorry for having offended you, and I detest all my sins because of your just punishments, but most of all because they offend you, my God, who are all good and deserving of all my love. I firmly resolve, with the help of your grace, to do penance, to sin no more, and to avoid the occasions of sin.
